Become a Customer
Sportsman Cap & Bag is a wholesale-only distributor. Accounts are open to businesses buying for resale — distributors, decorators, promotional products firms, and retailers. We do not sell to the general public.
How to open an account
- Download the New Customer Information Form. The PDF is fillable, so you can type directly into it and save — no printing required.
- Complete every section. Include your business details, billing and shipping addresses, and your resale certificate information.
- Email the completed form to newcustomer@headwear.com, attaching a copy of your state-issued Resale Certificate.

What you will need
- Business information — legal business name, address, phone, and billing contact
- Type of business and how long you have been in operation
- A state-issued Resale Certificate, including the certificate number, issuing state, and expiration date
- Billing and shipping addresses, plus the email addresses for order confirmations, invoices, and statements
- Your ASI or PPAI number, if you have one
A valid Resale Certificate is required before any tax-exempt order can be processed. We will review your application and follow up within two business days.
